Role Description The Service Coordinator is a full-time, remote role responsible for coordinating and overseeing disability support services to ensure customers receive timely, high-quality, and person-centered support. Day-to-day tasks include managing customer referrals, assessing needs, developing service plans, and coordinating service delivery in line with NDIS requirements and Northcott’s values. The role involves regular communication with customers, families, carers, and internal teams, maintaining accurate records, and monitoring service outcomes. The Service Coordinator collaborates with multidisciplinary staff, resolves issues, and escalates risks as needed to support safe and effective service provision. This role also contributes to continuous improvement by identifying service gaps, supporting innovation in service design, and promoting inclusive practices.
Qualifications
- Demonstrated skills in customer service, stakeholder engagement, and relationship management to support people with disability, families, and carers.
- Experience in service coordination, case management, or care planning within disability, community, or human services settings.
- Strong organizational, time management, and prioritization skills to manage caseloads, schedules, and documentation effectively.
- Proficiency in written and verbal communication, including report writing, documentation, and professional correspondence.
- Ability to work independently in a remote environment while collaborating effectively with multidisciplinary teams.
- Knowledge of the NDIS, disability sector, and person-centered practice; experience with NDIS plans or similar funding models is highly regarded.
- Competence with digital tools and systems, including case management software, online communication platforms, and standard office applications.
- Relevant tertiary qualification in social work, human services, disability, psychology, or a related discipline, or equivalent experience.
- Commitment to inclusive practice, human rights, and safeguarding, with the ability to uphold Northcott’s values of being innovative, respectful, and brave.
